1. Yes, absolutely. PitStop can check for the size of any of the PDF boxes (trim, crop, bleed, art and mediabox). So you could easily customize a PDF Profile so that it errors (or warns) on a trimbox not being A4.
2. Depends on how exactly your document is formed to be honest. I would send an example file and your question to
support@enfocus.com for this one so that they can see whether this is possible.
3. As far as I can see you can't use any of the predefined checks in the PDF Profile (unless you know beforehand how many spot colors you're expecting), but you can create an Action List to check for this (and use that Action List from within the PDF Profile). Again, support will be able to help you with the details.
