Catching Printing Defects

Hi all,
We have a Heidelberg and Man Roland. We are looking for different techniques or processess for catching printing defects in print press sheets for the Press Operators and Quality Control People. Too many things are going unnoticed.
Any suggestions would be appreciated... checklists alone are not doing the trick
