Interview Questions for a Graphic Designer
Maybe this will help some. Personally, I think flexibility is very important in a designer. Also a good look at a portfolio is good.

My company does a Wonderlic test and DiSC profile on all new hires and it's surprisingly helpful. It doesn't apply to any specific position, but just the general background and personality of the person and how the are capable of learning and how they interact with their environment and other people. When my boss and I went over the DiSC profiles of everyone in our shop it was amazing how accurate it is.
