We simply use cut press sheets without color match, and ink reduced on an older Epson 7800. We then manually flip the sheets and use the guides on the machine for registration. We can back forms up close enough for dylux/mock-up reasons. We do see some skew when two different operators load the different sides of the sheet though.
We had a HP DesignJet 1000 with a Techsage SpinJet 1000 attachment up until 5 years ago.
Now we have a Preproofer from Digital Imaging. It is 2 EPSON 9XXX stacked on top of each other, the bottom unit rotated 180 degrees from the top unit. The top unit is fed from a roll. It prints the sheet and cuts it. The sheet slides into the bottom unit as a sheet and the back-side is printed.

Usually no more than 1/32". But like all devices of this nature it has its good days and bad days.
The newer model is better because it uses a camera system for back-up registration.
We use a Canon Impoproof iPF8000s to produce 2 sided dylux. Registration isn't normally an issue, but like any piece of equipment, it can have its moments. I would say 95+% of the time there is no issue. The time savings made this a no-brainer decision 3 years ago.
