I'm sure I read somewhere that Photoshop Save As doesn't degrade jpeg unless you've edited some pixels. I'm interested if there was a difference between saves 9 and 10. it could be that all the difference was generated at the 1st save.

Flattening of the placed PDFs gets handled by JAWS on import, I don't think the flattening resolution for Export would make any difference to the quality. Quark specifically said NOT to re-place a native transparent PDF that it generated back into a quark layout but rather flatten it first.

I've seen similar corruptions from InDesign and Quark, when you remake the PDF they are gone. If there is any anti-virus software I'd turn it off, if it gets worse I'd try and re-format or change the Hard Disk

Select the pages you need to add marks to. Template>Modify Template Page>Add Crop Marks often its best here, to not select add to outside only. Now hide your Pages and you should be able to click drag (over the whole mark) to select all the marks that you don't want and hit the delete key...

had a similar file with same symptoms a while back, nothing composite worked here at all, in the end I printed separated postscript and left it to the RIP to recombine to composite PDF.
