I would try some rubber roller cleaner/Rejuvenator to clean the rollers and give them a little tack. Don't forget to clean the separator too.

All I can say is.. 6 or 7 links I tried.. for the service manual, my Avast Antivirus blocked it warning of infection. Perhaps you knew this already. I did get this.. User/Installation manual pdf. May not help you. :(

Increase clamp pressure. Or sharpen the blade. Or Both. A dull blade will "drag" sheets.

Check that each page is the same size. Possibly different by a fraction? With Quite Imp. You would have to "Trim & Shift" and make all pages the same size first. But.. you can also shift odds and evens, single or range of page, independently.

QUITE Imposing. Many tools, fairly simple to implement. Just think through your steps logically. Works with most versions of Acrobat Pro. Step and Repeat. Instant Booklet signature. Creep Adjust. etc.. Save Processes of common jobs.

Something like Planet Press (Obsolete/Replaced w/Connect?) should be able to process your static and variable pages into one big file. Just don't know how you'd program cover stocks inline. Prisma has a lot of shortcomings.

I do that with PDFs in Acrobat Pro 11, and Quite imposing. One up for client. Multiup for print.

Funny.. My (one of) vendor was Lewis. Bought by Lindenmeyr.

I'd push that temp up another 30 + deg. Because of the thicker film. Also recognize that the first couple sheets will cool the small rollers down considerably, and it take a little time for the temp to catch up. So you may have 2-3 or 5-6 that did not get the proper temperature. All LOOKS good...

Outsourcing Lamination is also an option ! Just be sure they offer a "de-curl" function on single sided covers. Then you don't have that big nut to pay off.
