What equipment are you using? Can the sheets come off flat, trim top and bottom before they are folded and stitched? Face trim only, after they are folded.
I have both a ricoh and canon with a server folder assigned to scan to, for 1 touch scanning. years ago a tech set up the canon, it was easy to save the settings on the ricoh so I did it myself.
The half where it doesn't work, I can't see the status screen from the monitor on the printer. I get a *not programmed* message. I'll need to cancel and restart the run from CW?
The "cutting guide layer" in indesign isn't in your screen captures. That's were something is happening. my guess is guide set to correctly sized page and every 4th sheet is bigger. Give us a screen capture of the layout and media tabs after checking you page sizes in indesign.

If it is "sometimes" it is in the job settings. When a job does it and you reprint it by clicking on the job in the printed folder of the fiery, changing the delivery try option at that point will fix it? I don't think it will. You have auto rotate or page orientation going on somewhere.
My service tech wouldn't leave before this was fixed or escalated! I just can't wrap my head around a mechanical problem being consistent enough to print every 4th page correctly, just rotating it 180. And the only symptom? Has to be software related. Are you letting it auto rotate/center in...
I see this when customer supplied files are readers spreads instead of printers spreads., Pages are reordered, not cropped, and bleed added or deleted incorrectly.
I would like to know you opinion also. I look at these from time to time as a step in between a ricoh 7210 with gold or our HEIDELBERG windmill leafing press.
