DPB-500 binder: Variable cover skew and mismatch between score and fold — looking for experienced insight

Hoping to tap into some real-world bindery experience here...

We've been running a Duplo DPB-500 for five years now, and we've had two long-standing issues that multiple service visits haven't resolved. We're trying to determine if this is a known mechanical characteristic of the machine.

Cover skew:
• Cover skew varies from run to run, anywhere from 0° to 0.25°
• The skew is small but measurable, and shows up over the length of the spine
• We've implemented a counter-skew in the design phase that centers most output, but it doesn't eliminate variance

Scoring mismatch:
• The spine score and the fold do not consistently coincide
• The score is typically off by 1/16", resulting in a less crisp fold

These behaviors have existed since it was first installed. I'm interested in hearing from anyone who has run, installed, or refurbished a DPB-500.

Everything has a home position for setup. Maybe the belts that drive the fold guides have some slop in them allowing to vary the stop position. Is the feed tray square to the score rollers? Or the feed guides?
