Open boxes showing up in random places

Okay, I'm stumped on this one. We are printing a file in which we get random open boxes in the middle of text(Helvetica) throughout a print job. We had this problem last year and assumed it was a problem with the from end RIP(Splash). This year we are running it on a different machine and different RIP. Still the same problem.

The boxes are not consistently showing up on the same page or same consistency.

It sounds like subset font on PS rip issue.
You can post PDF and someone will check it.

Was the file created in InDesign CS4 with a Quark pdf placed in it?

I will post a reply in a bit. The file was created in CS3 and we placed the pdfs into Indesign(CS3) and printed that way.

try turning off ligatures in all of the text boxes of the indesign file that made the created pdf that was placed into the new one and recreate it. make sure you embed all fonts as well.
