QM46-2 gear damage

We have had significant damage to the gears on non-operator side of Unit 2 of our QM46-2 press....metal shavings everywhere. We purchased a second press and within 30 days the exact same gears were destroyed on that press as well. Has anyone experienced this and what did you find the cause to be? We have already made sure the floor is level, the unit has substantial grease on the gears, no new chemicals, plates, or other materials, we have very experienced press operators...not sure what could cause the damage, but I need to figure it out before we invest in more expensive repairs.

You must check the meshing (tooth in tooth space) of cylinder gear to each other..
The machine some time, must be repair, and was bad adjustment and with that Result.
Need a mechanical o service manual.
