Three words:

Device Link Profiles.

Now that Photoshop CS4 supports device links, it's a relatively easy proposition to perform device link conversions. All you need is the link itself or software to create the device link.

Callas pdfToolbox is not very expensive and as you said supports device link profiles for PDF's. If you're dealing with just images, as in PhotoShop, then PhotoShop is the way to go.

Is "Press Repurposing" a separate task processor? There's a place to use device link profiles in the Render TP.

I usually use the conversion tool in Acrobat and select to preserve blacks. Acrobat 9 does this pretty well. I do it after I save the file as a PDF. PitStop might have some helps as well.

Press repurposing requires you to have a Colour Tune licence and is a feature of Apogee 6. The black chanel is maintained as far as possible but the TAC of the output intent is honoured.
There is also a product from agfa Called inksave wich is a gives you more controm´l over the repurposing and creates a device link on the fly according to your needs, In Inksave the GCR settings are split between the general image, midtones etc, and you can also specify if you want higher GCR in near neutrals or if you want the aggresive GCR to be active even in saturated colours.
