Proof and print file preparation with GRACol profile

Just was wondering what the best practices for outputting and proofing files with a GRACol ICC to keep everything color consistent.

Currently have all working spaces in Adobe products set to GRACol2013, but that will only apply it in the program correct? What is the best way to output proofs and print files correctly so that the gracol profile is applied?
I believe that the color profile must be embedded for it to view correctly in the proof, if it is embedded will all pdf viewers honor the profile?
What about pdfs that are not put through Indesign, what is the best way to convert to the GRACol profile?

Any help would be appreciated.
