Source for insurance claim envelopes?

I am looking for a source for insurance claim envelopes for a client. They have had their current supply for so long and have no record of where they purchased the last order. Specifically - 4 1/2 x 9 left window with interior tint and fold to seal flaps.
I've asked our current vendors and searched online but see exactly what they want. The self seal option is important to them. Does anyone have a source or suggestion?

Western States Envelope and Envelope Printery should both be able to manufacture that, even if they don't have existing dies.
