Xerox SquareFold Trimmer skips square-fold randomly

We run a ton of 11x17 to 8.5x11 books through our Xerox PrimeLink B9125 with SquareFold Trimmer and I'm getting this weird issue where it randomly just skips the square folding step. It's so infrequent (once or twice per 100 books) I can't really troubleshoot it properly. It always happens in between two normally finished books without any break in printing so there's no consistent trigger for it, like it running out of paper or something. Anyone experienced this issue before?
 
We (likely) have the exact square-fold/face trimmer on our Xerox as you do, they are pretty universal, all made for Fuji, with the Xerox name slapped on them.

We have the same random issues, and we often can attribute it to dirty sensors and/or trim crossing the sensor at the same time as the booklet. Not out of sync enough to the booklet to trigger a "jam" but close enough that the booklet is miss-timed by maybe a millimeter or so. Could also be the booklet slipping on the drive surfaces (belts or rollers)? I find that if I take a vacuum/shop-vac to the inside of the unit a couple of times a year, especially down in the bottom on the way to the exit, it helps a ton.

If you need help opening the unit, I can write something up. its pretty easy when idle, stick a booklet back inside of the exit and inch or so (2-3cm). open the trim waste bin, and close, this will trigger a false jam and allow you to open the covers.

Thank you, I was wondering how to get it open without a real jam. I'll give it a good clean out and see if that helps.

I did that little trick right in front of a Xerox tech and you would have thought I did some kind of voodoo... he was like "HOW DID YOU DO THAT?!?" LOL

We used to have the Xerox square fold trimmer and I REALLY wish I had known this trick. It would have made life so much easier!!
