Did you check the PDF if that problem pantone spot had been inadvertently set to overprint? The RIP setting Composite overprint could affect this as well.

I did look at it in Illustrator at one of the problem pages, it was not set to overprint. I really could not find anything wonky or bizarre about it. No effects, gradients, transparencies. just a box, rounded corners and a 20% tint of the pantone colour.
