All the suggestions above are correct however we found that most of the time the problem was caused by the Fuser Heat Belt Slip Sheet which is a Teflon strip that is fitted at the pressure point to the pressure roller.
This either gets dirty or tears away from the fixing points and folds over. There seemed to be a quality issue with the original Xerox slip sheets for a period of time.
The slip sheets are cheap and available in 10 packs and reasonably easy to replace and 90% of the time solved the problem. We managed to get original Fuji slip sheets which were longer lasting.
